This kind of playthrough not only restricts access to specific rewards but, if you engage in a Dark Urge storyline, you might find yourself in a situation where you have to kill Alfira, depending on her survival status when a crucial scene occurs.
It’s possible to save her, but it typically requires a series of actions that can feel frustrating. Furthermore, some gamers have noted that simply knocking her unconscious isn’t always effective. Fortunately, a user-friendly solution is now available thanks to the “Save Alfira” mod developed by TLG53787.

This mod’s functionality is straightforward; it preserves Alfira’s life by seamlessly substituting her with Quil during the pivotal night scene associated with the Dark Urge Origin story.
For those unfamiliar, Quil serves as a backup bard NPC who appears in camp should Alfira perish prior to the murder scene connected with the Dark Urge. Although your companions will still react to your character’s malevolent actions the following morning, you can at least ensure that Alfira survives within your playthrough.
It’s interesting to note that if Alfira survives until the very end of your Dark Urge playthrough, you will receive a letter from her, underscoring Larian’s commitment to making player choices impactful throughout the game.
